Answer:
The Group PTAN and Group NPI must be submitted on all EDI enrollment forms. Members of a group are not required to enroll, only the group PTAN. If you are a solo or individual provider and not a member of a group, submit your individual PTAN and NPI on all EDI enrollment forms.

Note: Providers enrolling for Railroad Medicare must use the PTAN assigned by Railroad Medicare.
